It has been agreed that all groups working under WSCB will be established by the WSCB, and will work to agreed terms of reference, with explicit lines of reporting, communication and accountability to WSCB. Chairs of sub-groups will be WSCB members. There are two principal types of sub-groups:
The individual agency safeguarding group has the primary responsibility for driving forward the safeguarding agenda, in particular the Section 11 responsibilities, within the individual agency or group of agencies, and monitoring how effectively this is being done. The multi-agency sub-groups will be functional sub-groups with the aim of ensuring both the co-ordination and effectiveness of local agencies in safeguarding and promoting the welfare of children in the Borough. WSCB has the following multi-agency sub-groups:
As of 1st April 2008 WSCB has a Child Death Overview Panel that has the role of reviewing all child deaths, in particular those children whose deaths were unexpected. There is a single point of contact (SPOC) who needs to be notified of all deaths of children in the borough, as well as of children who are resident to Wandsworth, but who have died outside of the borough. WSCB will convene time-limited working groups when the need arises to conduct particular pieces of work. Membership of these working groups will be determined by their function and will be invited from member agencies. The terms of reference for the sub-groups have been drawn up by each sub-group and confirmed by the full WSCB. All the sub-groups will be expected to integrate equality and diversity issues within their specific remits. The overall principles are that in all cases enquiries should seek to understand the reasons for the child's death, address the possible needs of the other children in the household, the needs of all family members and, also, consider any lessons to be learnt about how best to safeguard and promote the children's welfare in the future. All families should be treated with sensitivity, discretion and respect at all times, and professionals should approach their enquiries with an open mind. Wandsworth will follow the Rapid Response & Child Death Overview Panel Procedures drawn up by the London Safeguarding Children Board . Death Notification form for Child Death Overview Panel The Monitoring (Practice and Procedures; Evaluation and Quality Assurance) Sub-groupThis sub-group will ensure that WSCB carries out the general function of developing procedures and practice for safeguarding and promoting the welfare of children in Wandsworth as set out in regulation, and monitoring the effectiveness of both the procedures and the practice.Many of the relevant procedures are found in the London Child Protection Procedures. WSCB will ensure there is clarity about the action to be taken when there are concerns about a child's safety or welfare, including thresholds for intervention under the Children Act 1989 (both Section 17 and Section 47): This sub-group will also ensure that WSCB carries out the general function of monitoring and evaluating the effectiveness of services delivered by Wandsworth Council and WSCB members individually and collectively to safeguard and promote the welfare of children. Monitoring Sub-group's Terms of Reference Serious Case Review Sub-Committee & PanelThe Serious Case Review Panel will only be convened when any of the circumstances described in Working Together (2010) Chapter 8 are present, such as when a child dies or has been severely harmed and abuse or neglect are known or suspected to be a factor.The purpose of a Serious Case Review is to consider whether there are any lessons to be learned about the ways in which agencies work together to safeguard and promote the welfare of children. WSCB needs to identify clearly what those lessons are, how they will be acted upon, and what is expected to change as a result. As a potential consequence, WSCB will decide how to improve inter-agency working and better safeguard and promote the welfare of children following the Review. The Serious Case Review Sub-Committee will be convened to consider whether or not the criteria has been met for the WSCB to undertake a SCR and it will make a recommendation to the Chair of the WSCB accordingly. This sub-committee be chaired by the Assistant Director (Children's Specialist Services). If the Chair of the WSCB agrees that a Serious Case Review should be undertaken, then a Serious Case Review Panel will be convened. Membership of the Serious Case Review Panel will comprise as a minimum:
The Panel will be chaired by an Independent Chair and will also appoint an Independent Overview & Executive Summary Report Writer. The procedures for such Reviews are detailed in "Working Together" (2010) and the revised chapter within London Child Protection Procedures (Chapter 19). There may be cases where it will be valuable to conduct individual management reviews or smaller-scale audit of individual cases giving rise to concern but not meeting the criteria for a full serious case review. In such cases, arrangements will be made to share relevant findings with WSCB. The Serious Case Review Panel will consider, in the light of each case, the scope of the review process, and draw up clear terms of reference. Following completion of a Review WSCB will delegate day to day monitoring of the Action Plan to the Serious Case Review Panel and will receive regular detailed reports about progress on the Action Plan.
