Pathway plan

At or around your 16th birthday an assessment of your needs will be made with you. This will be the basis for preparing your Pathway Plan. Your Pathway Plan will map out your future dreams, hopes and wishes, and identifying goals along the way to realising your ambitions. The plan will state how the Children’s Service will meet your individual needs now you are older and preparing to leave care. It will include things about your education, accommodation, health and personal support.
