What is my care plan?
Every young person who is looked after should have a plan written down about where he or she is going to live, and who needs to do what to make sure that they are properly looked after. This is your care plan.
It gives information about you, about your hopes and plans for you and your future, and what everyone who is involved has to do to make sure the plan works. You should be involved in helping to write your care plan and you should get a copy of this once its written down.
Your care plan should cover
- Where you live.
- Your education.
- Your health, and how you can follow your hobbies and do things you like to do. It is about setting out what is to be done for you.
- It is about what you want and are expected to do as part of the Plan.
- What is discussed at a Review and the decisions made at the meeting are put in writing.
