Serious Case Review


Working Together to Safeguard Children (Chapter 8) sets out the purpose and process of Serious Case Reviews (SCR). SCR's are undertaken when a child dies (including suicide), and abuse or neglect is known or suspected to be a factor in the death.

The purposes of SCRs carried out under this guidance are to:
  • Establish what lessons are to be learned from the case about the way in which local professionals and organisations work individually and together to safeguard and promote the welfare of children;
  • Identify clearly what those lessons are both within and between agencies, how and within what timescales they will be acted on, and what is expected to change as a result; and
  • Improve intra- and inter-agency working and better safeguard and promote the welfare of children.
The processes through which SCR's are conducted are currently being reviewed within the Munro Review of Child Protection. In response to this, Wandsworth have decided to participate within the SCIE SCR pilot taking place within selected boroughs in London.

WSCB Serious Case Review - "Sarah"

Wandsworth completed a Serious Case Review in 2010 in relation to the death of three year old "Sarah" . This was a comprehensive look at our multi-agency safeguarding practice in respect of "Sarah" and the WSCB are in the process of implementing the learning points from the review.
